11/4/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival the home, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. Ms. Broadnax verbalized to QMHPA her morning was well. Intervention: QMHPA began the session by inquiring about Ms. Broadnaxâs current thoughts, feelings, and emotions. QMHPA supports daily compliance with her medications so that she can continue her consistently. QMHPA discussed with Ms. Broadnax on strengthening her communication skills when she may feel frustrated verses being verbally and or physically abusive. QMHPA supports practicing breathing techniques daily, developing daily affirmations and journaling daily as skill builders she can use in coping with her feelings of anxiety and paranoia. QMHPA supports improving her mental health symptoms through identifying her triggers that creates her anger and isolation. QMHPA reinforced utilizing her support system to help her decrease her depression, utilize journaling to help her decrease her stress from 5 days to 3 days per week. QMHPA supports practicing eye contact, voice tones and body languages with a family member 1-2 times per week so that she can improve her communication skills. Response: Ms. Broadnax appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Upon arrival Ms. Broadnax was talking on her cell phone in a very loud tone. Ms. Broadnax apologized for being rude. Ms. Broadnax stated, âSometimes you just got to let people know you mean business. Ms. Broadnax was dressed, and Ms. Broadnax was prepared for the session. Ms. Broadnax shared that she was not able to avoid all the conflicts she encountered but she managed to identify two that she felt she was on top of. Ms. Broadnax was supported for being able to redirect a behavior through avoiding any verbal or physical confrontation. Ms. Broadnax shared that she can contribute the positive behaviors to being medication compliant. Plan: QMHPA will continue to teach Ms. Broadnax how to schedule as well as manage appointments and educate her regarding the positive impact of medication through various skill building activities 4-5 times (Monthly).
